Names – and the names of spouses, significant others, kids, etc. that will be flying with us.

Address – Please include city, state, and zip.

Home Phone – Self-explanatory. Please include the area code.

Work Phone – Optional. We only want it if you can be contacted at work. We don't want to get anyone in trouble. Again, please include the area code.

E-mail Address – Required. We hope to do all of our communication through E-mail.

Your Work Schedule – Only if it deviates from the normal 8 hour a day, Monday through Friday. This will be used for setting up our flying schedules which will be every other weekend.

                    Closest Major Cross Streets – For fly scheduling purposes. It's a big city.

Your Favorite Flying Field – Close to your house, that will support several fliers, for our flying schedule.
